Darryl S. Rich is currently a surveyor with The Joint Commission in the hospital, home care, and ambulatory accreditation programs.  In addition, Darryl serves as an internal consultant and external representative for The Joint Commission on various issues related to pharmacy and medication management in the accreditation programs and standards interpretation group. He currently sits on The Joint Commission International Center for Patient Safety Medication Safety Expert Panel.

Prior to accepting a position with The Joint Commission in 1993, Dr. Rich was Director of Pharmacy Services at Boston University Medical Center and Clinical Assistant Professor of Pharmacy at Northeastern University.  Dr. Rich received his doctor of clinical pharmacy degree from the University of California at San Francisco, and his M.B.A. in Health Care Management from Bryant University in Rhode Island.

Dr. Rich is a past President of the New England Council of Health-System Pharmacists and a fellow of ASHP.  He has received numerous awards including the 2000 Cheers award by the Institute for Safe Medication Practices.  In 2004, Dr. Rich was honored with a Certificate of Appreciation from the Board of ASHP for his work on medication safety at The Joint Commission.  Dr. Rich has delivered over 460 invited professional presentations and 74 publications in refereed journals, including 8 books and 4 video series, and previously had a column on Joint Commission accreditation in the journal, HospitalPharmacy.

Howard I. Cohen is Corporate Director of Pharmacy Services for Universal Health Services, Inc, one of the nation’s largest healthcare management companies. Universal Health Services, Inc. operates acute care hospitals, behavioral health facilities and ambulatory centers nationwide. Mr. Cohen previously served as the Director of the Department of Pharmacy Services at Thomas Jefferson University Hospitals. He also holds the position of adjunct faculty at Temple University College of Pharmacy.

Mr. Cohen earned his Bachelor of Science degree in pharmacy from the University of Kansas School of Pharmacy in Lawrence, Kansas, and his Master of Science degree in hospital pharmacy administration from Temple University School of Pharmacy in Philadelphia, Pennsylvania. He completed a fellowship at the Wharton School of Business - Leonard Davis Institute at the University of Pennsylvania.

A fellow of the American Society of Health-System Pharmacists, Mr. Cohen is also a member of the American Pharmaceutical Association and several state and local professional societies. Mr. Cohen is the Editorial Advisor for the Pharmacy Net Guide, and is an experienced speaker and advisor on various pharmacy and hospital decision-making topics.

Mark Flyer is currently Vice Chairman of the Department of Radiology at Maimonides Medical Center in New York.  He is also Assistant Professor of Radiology at Mount Sinai School of Medicine and Medical Center.  Previously Dr. Flyer was Director of Body Imaging at Mount Sinai and Director of MRI at SUNY Downstate-Brooklyn and Veterans Medical Center for three years.

Dr. Flyer received his medical degree from Texas A&M University College of Medicine.  He completed a Diagnostic Radiology Residency at SUNY Downstate Health Science Center at Brooklyn.  Dr. Flyer then completed an MRI fellowship at New York University Medical Center.

Dr. Flyer has been actively involved in MRI for many years and is specifically interested in Body and Cardiovascular MRI.  He is also very interested in MDCT, 3D imaging and CT/MR contrast agents.  Lecturing nationally and locally, Dr. Flyer has helped to develop many CME programs for MR Technologists.  He has also co-directed several CME courses for practicing radiologists and physicians.

Dr. Flyer is actively involved in radiology resident education and participates on the education and admission committee for the radiology residency program at Maimonides Medical Center.
